What to expect

- 1.Fill out the application. Enter the registrant's full legal name, date of birth, city of birth, and your relationship to the person named on the certificate.
- 2.Complete a notarized application. Texas requires a notarized application verifying your authorized relationship to the certificate holder. Instructions for notarization are included with your application.
- 3.Submit payment. The total cost includes a $22 DSHS state fee plus a service fee. See the full fees breakdown for details.
- 4.Receive your certificate by mail. Once DSHS processes your application, the certified copy is mailed to the address you provide. See our order status page for current processing times.
